Before Filling Out Your Form . . . follow these instructions
to prepare your Browser to help assure proper data transmission.

In Internet Explorer:
To clean your Browser Caches

  • Click on /Tools
  • On general tab Click on /Internet Options
  • Under 'Browsing History', click on 'Delete', then make sure 'Temporary Internet Files' are checked  then press 'Delete'
  • Also under 'Browsing History' click on 'Settings' and make sure 'Every time you start Internet Explorer' is checked off, then click 'OK'


Assure your Form Data Persistence

  • Click on /Tools
  • Click on /Internet Options
  • Click the 'Security' tab
  • Click 'Custom Level'
  • Find the 'Miscellaneous' section below
  • Find 'Userdata persistence'
  • Click 'Enable', then click 'OK', then 'OK' again
  • Close down, then restart your Internet Explorer for the changes to take effect

In Mozilla Firefox:
Do the following

  • Click on /Tools
  • Click on /Options
  • Click on /Privacy
  • Make sure settings for Firefox are set to "Use custom settings for history"
  • Click "Remember search and form history"
  • Click "Clear history when Firefox closes"
  • Under Settings make sure "Form & Search History" is not checked

In Netscape:
Clean your Browser Caches and Assure Form Data Persistence

  • Click /Edit /Preferences
  • Double click Advanced
  • Click Cache
  • Set the Memory Cache to at least 1024KB
  • Set the Disk Cache to a minimum of 7680KB
  • Clear both Caches

 Tips for Navigation and Viewing
You can navigate either:

  • by using your Tab key to move from one question to another and your up/down Arrow keys to select your answer, or
  • by using your Mouse and Scroll Bar and clicking in the square next to your answer.

If the type size appears too small or too large, you can change the setting in your browser.

  • In Internet Explorer, on the top menu, click /View /Text Size and select Larger or Smaller.
  • In Firefox, on the top menu, click /View /Zoom In to increase size
  • In Netscape, on the top menu, click /View /Text Size and select Larger or Smaller.

Remove any Toolbars you don't need in order to see more screen.

  • Click on /View /Toolbars and uncheck any you don't need. Or, click on /View /Full Screen.In Firefox to return to normal viewing screen click rectangle in upper right hand corner.
